The ‘Youth Transition’ project in Slovenia helps thousands of students with disabilities enter higher education or find work. This four-year project helps them find education and job opportunities and provides career guidance. Youth Transition involves everyone in the process: families, professional staff, teachers, employers and a range of professional organisations in the fields of health and social care.
