Integrated care is essential for people living with rare diseases to attain the highest standard of health and well-being, and to achieve equal opportunities, rights and full participation in society. However, integrated care is not yet a reality for most people living with rare diseases and their family members, who experience significant care coordination gaps. For example, 65% have to visit different health, social and local services in a short period of time, while 67% find that these services communicate badly with each other. In addition, 7 in 10 find that organising care is time-consuming and 6 in 10 find care hard to manage.
INNOVCare aimed at bridging the gaps between health, social and local services to improve care of people living with rare and complex conditions.
The project mapped the gaps and unmet needs, piloted a successful case management model, created an European Network of Resource Centres that support people with complex conditions, and outlined policy recommendations for achieving effective integrated health and social care.
The actions of the project were delivered in partnership between health, social and other support services at national, regional and local levels. In addition, the project's case management pilot was evaluated based on semi-randomised trials and interviews.
